Market Research Survey Template

The odd one out, because it asks about a market rather than about you. What the survey is actually about comes from the title and description you write, which every survey has, so the questions themselves stay usable as they are. Note the timeframe on the switching question: without one, people answer about their intentions in principle, which is always more positive than their behaviour.
