Employee Engagement Survey Template

The most sensitive of the six, and written accordingly. Only two questions are required, and every question someone might not want to answer is optional. The tenure bands are deliberately broad: narrower ones make people identifiable in a small team, which quietly destroys the honesty of every answer above them.
