Employee Exit Survey Template
Exit surveys cannot really be anonymous: people usually leave one at a time, so the date alone can identify someone. The survey says confidential rather than anonymous for that reason, and the questions avoid anything that narrows it further, with no tenure, team or role. The main reason is a fixed list so it can be counted across everyone who leaves; the free text after it is where the useful detail lives.
